If you would like to see Dr. Nelson:

• Fill Form Out, or call to make an appointment.
• Dr. Nelson will do a complete and thorough examination.
• Any required X-rays will be taken.
• He will review all your findings with you.
• He will then design a treatment plan to get you well!
• All fees will be discussed before any care is given. We will never surprise you with a bill.

Paying for your care:

• Auto Accident injuries are covered 100%.
• Work Comp injuries are covered 100% for 12 weeks.
• Medicare does not pay for exams and x-rays so the first visit will be $100. Medicare then covers 80% for up to 12 weeks.
  • Note: We are in the process of becoming a Medicare provider.  Please fill out the form and we will contact you.
• Regular health insurance – We are not in-network and do not accept any as they rarely pay anything anyway. The first visit fee is $100.
• No insurance? Great, the first visit fee is $100 then we will create an affordable plan to get you feeling great again!
